This is a chance to help shape Calderdale Council’s new Carers Assessment form. They would like to hear your thoughts on how they can change the form for a better experience. They would then like to take the ideas to a workshop style event during Carers Week in June to share with others.
If you would like to contribute, please do get in touch and the invite includes the opportunity to stay on after the meeting to enjoy lunch – please do let them know if you can make it so they can arrange catering. If you have any special dietary requirements or allergies please do include that information in your response.
