Your views and experiences are important in helping Calderdale and Huddersfield NHS Foundation Trust (CHFT) to improve the Transition Service for young people and their families with neuro-disabilities. Transition is the purposeful and planned process of supporting young people to move from children to adults’ services. This process usually starts on or around their 14th birthday, where the professionals who work with young people should have been starting to talk with you as a family about transition and what this might look like for you. We think it’s really important to listen to all our service users, patients and carers and would like to hear what you think about the transitioning process at CHFT.
