What is the Covid-19 Response service?
Age UK’s Covid-19 response is a collection of services provided for up to 6 weeks to support someone who has been in hospital and/or to prevent a hospital readmission.
Who would benefit from the programme?
The programme is aimed at people who:
- Are over 50 years old or under 50 with a disability
- Have been discharged from hospital and risk being readmitted within 6 weeks
- Are at risk of being admitted to hospital but could be safely cared for at home with additional support
What is included in the support package?
Each package of care will be personalised to the needs of that individual, it can include a range of the following services as required:
Settling in service
Environmental check (including falls assessment)
Installation of a key safe
Pre-booked transport to medical appointments (GP, clinic, dentist etc)
Information, advice and signposting
Essential shopping delivery
Advice on mobility aids
Regular welfare calls
They may choose to include other things depending on demand

Who can make a referral?
Anyone can make a referral, e.g. voluntary sector, community-based health and care services (e.g. Local, GPs), ambulance, family, self-referral, police, social care, fire and rescue, Hospital (discharge, ED or other)
When is the service available?
Monday to Friday, between 9am and 5pm
