Autism & Learning Disability “Grab & Go” Guide

If you need to go to hospital it is useful to have a Grab and Go guide about your support needs. It is a bit like a hospital passport. It has information about you on it that doctors need- that they can read really quickly.

It tells them if you are autistic or have a learning disability- and what you need to support you – for example what reasonable adjustments you need.  Fill it in as soon as you can and take it to the hospital with you, if you get ill.

You will still take your hospital passport with you too, if you have one.

If you don’t have a hospital passport you could fill in a Grab and Go . Or some hospitals have their own passport that you can use- you could talk to your hospital Learning Disability Nurse or Patient Advice and Liaison Service (PALS) to find out.
